This deliverable has three parts, an implementation narrative, a monitoring plan, and a SWOT analysis — that together form the blueprint for putting your solution into practice. You will also use your implementation plan to complete a Gantt Chart that visually maps out the key activities, responsible parties, and milestones over the six-month period.
Assignment Steps Part 1: Implementation Narrative An implementation plan is a roadmap that explains how a project will be carried out. It translates your ideas and design into concrete actions, detailing what will happen, who will do it, when it will occur, and how progress will be measured.
1. Main Goals and Focus • What are the main goals of your project in the first six months of implementation? • What specific change or impact are you hoping to see as a result of your efforts? • How will you know that progress is being made toward these goals?
2. Objectives • What 2–4 specific, measurable, and time-bound objectives will guide your work during this period? • How do these objectives connect to your overall project goals and design? • What evidence or indicators will show that you’ve met each objective? • Which objectives are most feasible to accomplish within the first six months, given your organization’s current capacity?
3. Approach and Key Activities • What are the key activities or strategies you’ll implement to achieve your objectives? • Who will be responsible for leading or carrying out each activity? • What community partners, agencies, or collaborators will be involved, and how will they contribute?
4. Timeline and Milestones • How will you phase your activities over the first six months? • What are the most important milestones you expect to reach, and by when?
5. Anticipated Outcomes • What short-term results do you expect to see by the end of the six months? • How will these results move your project closer to your long-term vision?
2 Part 2: Gantt Chart After completing your Implementation Narrative, translate your plan into a visual Gantt chart using the provided template. Your Gantt chart should visually mirror your implementation plan – it is the same story told in a timeline format. Your Gantt chart should: • List each major activity or task aligned to your objectives. • Identify the person or partner responsible for completing it. • Include start and end dates for each activity. • Highlight milestones (key checkpoints or deliverables).
Part 3: Monitoring Plan A monitoring plan explains how you will track progress toward your goals. It specifies what data will be collected, who will collect it, how often it will be reviewed and how findings will be used to make improvements along the way. 1. Performance Measures • What data will you collect to track progress toward your goals? • Who will collect it, and how will it be stored or analyzed? • If you collect personally identifiable information (PII), how will you protect it? 2. Reporting and Feedback • Who needs updates on your progress (e.g., funders, community partners, leadership)? • How and how often will you report back? • How will feedback or data findings be used to adjust your approach?
Part 4: SWOT Analysis A SWOT analysis helps you assess your readiness for implementation by examining internal and external factors that may influence your success. 1. Strengths • What assets, skills or resources does your organization or project have that give it an advantage? 2. Weaknesses • What internal challenges, limitations or gaps could hinder success? 3. Opportunities • What external trends, partnerships or community conditions could help your project succeed? 4. Threats • What external risks, barriers, or competition could pose challenges?
Things to Avoid • Writing a list of tasks without describing the “why” or “how” behind them. • Setting vague or overly ambitious goals. • Skipping performance tracking or feedback mechanisms. • Overlooking internal weaknesses or external threats in your SWOT
